Sam is just a made-up fictional child to show how the Individualized Education Program (IEP) is a team approach.
Fall 2016
Activities aligned with Daily Living Skills
Many
thoughts are considered as the team is in preparation mode for Sam go to
Kindergarten next year. At has IEP
renewal in September, which I was able to do as the Itinerant Teacher. The team had many goals for him as follows:
1.
Learning goas need to be set-up with the IEP team and family.
2.
The team would like to see Sam continue to work on his
Fine Skills, because he is currently getting Occupational Therapy.
3.
He will do activities with the Occupational
Therapist and Itinerant in play form to improve his fine motor, without getting
frustrated.
4.
The team would like to see Sam continue to improve of his
oral communication skills, with the Speech and Language Pathologist, Itinerant
Teacher, and all staff.
5.
His Mom would like to
see him get self-help skills. Mom mentioned concerns
with social-emotional.
6.
Make
flash cards and other manipulatives for the family to work on at home.
7.
Send
notes with directions if we are unable to talk in person or on the phone.
8.
The team decided to
address this at the next ETR meeting, however he was
given a
communication goal that worked on sharing with peers and teachers.
This
seemed to be appropriate to all team members.
Winter 2017
1.
Everyone
works with Sam and communicates home with notes and phone calls as needed.
2.
The family is also given progress reports
within the school calendar.
3.
His transition planning meeting is scheduled
for later this month. Since his ETR
isn’t due there will not be a full evaluation done. Because Sam is on an IEP the team is familiar
with him and his progress.
4.
He is also having state required general
education assessments done in the classroom setting.
5.
Check-in with the teacher and family to see
if you can provide more support with notes, face-to-face, and phone calls.
Spring 2017
1. In the spring the team will come back together
and make-up a IEP that is
appropriate for Kindergarten
in a school age setting.
2. Many students will no longer have their IEP,
but Sam is likely to stay on one.
3.
Revisit learning goals and evaluate current needs with the IEP team and
family.
4.
Equip the family with items that will help Sam stay sharp during
the summer.
5.
Use dramatic play, station time, and all classroom activities in a
“getting ready for Kindergarten” theme.
6.
Model upcoming events.

Summer 2017
1. Sam’s family is very involved in working with
him and they will be equipped with
ideas from the team, their research.
2. Sam’s doctor’s visits on ways to help Sam be
the best form of himself.
Projected Completion Date:
His
Evaluation Team Report is due for renewal is 9/21/2017 and his current
Individualized Educational Plan is due 9/28/2017.
